Position Summary
The general purpose of this position is to contribute to the provision of strategic services to children living in vulnerable environments and their caregivers, to address important barriers to access to services, use, and adherence to increase the impact of service delivery, to continue progress towards the goals of 95 -95-95, and improve. health and social outcomes among children living in vulnerable environments and their families.

The incumbent will ensure an effective and efficient referral system with two-way linkages, improved case detection, and linkages to ART and Viral Burden suppression among Children and Youth Living with HIV/AIDS.

  • Coordination and Representation:
  • Represent AWG and coordinate with the Council Health Management Team regarding HIV and health related activities in the Council Team.
  • Conducting monthly coordination meetings with Care and Treatment Centers (CTC) in the implementation area to support shared confidentiality, facilitate case conferencing,
  • improve the two-way referral system, and ensure HIV-positive beneficiaries receive needed services in the Team of the Council.
  • Support and Management:
  • Supporting Early Child Development (ECD) in targeted health facilities, including regular monitoring and support management in the Council Team.
  • Tools to help maintain an improved enrollment register showing the flow of CLHIV enrollment in CTCs in the Council Team.
  • Assist Community Workers (CCWs) to work with high-volume pediatric CTCs to locate OVCs with HIV who miss appointments, enroll them in the USAID KIZAZI HODARI SOUTHERN PROJECT, and connect them to services.
  • Assist CCWs to conduct HIV risk assessment for OVC during case management; refer and connect OVC at risk to HIV Testing and Counseling (HTC).
  • Program Implementation:
    Schedule quarterly Reproductive Health Education for Girls and Young Girls (AGYW), DREAMS Girls in schools, and organize access to HTC for youth with risky behaviors in the Council Team.
  • Collaborate with Health Center Implementation Partners to launch additional HIV curriculum for CCWs and ensure CCWs provide appropriate support to OVC and caregivers living with HIV.
  • Help CCWs strengthen the capacity of caregivers to support OVC living with HIV and ensure CCWs refer OVC and caregivers living with HIV to support PLHIV.
    Referral System Management:
  • Support the establishment of a two-way referral system to track beneficiaries through the continuum of HIV care as well as the completion of services for other health and social services.
  • Monitor the implementation, identify obstacles, evaluate the performance measures of the two-way appeal system; proactively provide solutions, perform quality improvement activities, and implement adjustments to improve referrals.
  • Participating in quarterly meetings to improve the quality of the council level with relevant stakeholders of the two-way appeal system; ensure that the steps of the meetings are implemented and the two-way appeal system is improved in the Council Team.
    Data Management and Reporting:
  • Work with the M&E officer to ensure all bilateral referral data is accurate and provided weekly; use statistics for decision making in the Council Team.
  • Lead the development of a list of health, nutrition and HIV services in the implementation area; update the directory at least once a year in the Council Team.
  • Assist CCW in running nutrition activities, including nutrition assessment, counseling and liaison with other nutrition service providers in the Council Team.
  • Provide ongoing support management to CCWs to ensure delivery of health related services and referrals and linkages to appropriate services.
  • Provide timely updates to the Program Manager for inclusion in quarterly, semi-annual, and annual reports to the Council Team.

Document lesson learned and best practices to share experiences and replicate across the Council Team.

Minimum Required Qualifications, Experience, and Skills
Education:
Required:
Diploma in Clinical Medicine.
Recommended:
Bachelor’s or advanced degree in Medicine or Nursing.
Skills:
Good understanding of the health systems of the Government of Tanzania.
Strong Monitoring and Evaluation (M&E) skills and experience in strategic reporting.
Excellent written and oral communication skills in Kiswahili and English, including excellent training skills for working with adult learners, especially at the community level.
Ability and willingness to work flexible hours as needed without constant or close supervision.
